If you have questions regarding your coverage, benefits, or claims, please feel free to contact us. Click on the Contact Us link for telephone numbers and addresses. You can also send an e-mail to us at customerservice@cnichs.com. Our standard hours of operation are Monday through Friday from 6:30am until 6:00pm (MST). During business hours, we respond to most e-mail within an hour.

Please remember, if you have to make any changes to your eligibility, such as adding or terminating coverage for yourself or a dependent, you need to check with your plan sponsor (usually your employer).

On-line access to claim, eligibility, and benefit information!

You can obtain claim, eligibility, and benefit information when it's convenient for you by using the internet. Simply enter your group number, and you will be provided with a link specific to your employer group.

You can use the site to view eligibility information, claim status, plan documents, and to search for a provider. You are also able to view and print copies of the Explanation of Benefits forms for paid claims. We also offer the option for electronic EOBs, so that you can receive notification via e-mail of a paid claim.
